Geri Care Health Services Private Limited (“Company”) is a private limited company, (formerly known as Geri Home Care Services Pvt Ltd), having its registered office at Old No.8, New No.21 First Avenue, Indira Nagar, Adyar Chennai, Tamil Nadu 600020 India. The Company offers a variety of healthcare services for senior citizens, ensuring comprehensive and effective healthcare for senior citizens including but not limited to doctor consultations, diagnostic services, physiotherapy and nursing services, pharmaceutical products, ambulance and emergency medical services, and hospital interventions through its own multispecialty hospitals.
The Website is designed to provide information about the health care and related services provided by the Company from its various Geriatric Multi speciality Hospitals, Assisted Living Centres, Geriatric Clinics, Diagnostic Labs, Physiotherapy, Mind & Memory Centres and Home Care Division. The website shall also facilitate users in booking appointments with Doctors for physical and tele consultations, and provide all information in availing a range of services including skilled nursing, Day Care services, Physiotherapy services, Nutrition & Dietary Advice to its patients, providing trained caregivers to look after elderly patients whether in their home or at our Assisted Living centres for Rehabilitation, Respite Care, Post Trauma Care, Post Hospitalisation Recovery, Palliative care and Specialised services for patients with Dementia etc. Additionally, the Website also processes payments for patients (the “Services”).

Definitions
“Registered User” means an individual who is either a Patient or their family member, and who creates an account on the Website by providing the patient’s personal information such as name of patient, phone number, and email address to accesses the Website to avail specialized healthcare services provided by the Company, on behalf of the patients. - “Patient” means a senior citizen or elderly person, who is in need of medical or healthcare services, including consultations, treatments, diagnostic procedures, or other forms of care from healthcare providers such as Medical Practitioners, nurses, specialists, or allied healthcare professionals.
- “Medical Practitioners” are individual doctors/geriatricians or doctors associated with Geri Care medical establishments, viz clinics, Assisted Living Centres, Geriatric hospitals, Pharmacies, Diagnostic Labs etc whom the Company has employed or set up or has collaborated with and who are listed on the Website to provide services to its Registered Users.
This Policy covers only data collected through the Website and not any other data collection or processing, including, without limitation, data collection practices of other mobile applications or web pages that are linked to the Website or data that is collected offline or through web sites, products, or services offered or belonging to third parties. Occasionally, reference would be made to this Policy in notices and consent requests related to specific surveys, ratings or other modalities present in the Website. By registering with the Website, You acknowledge Your acceptance of the terms of this Policy, expressly consent to the use and disclosure of Your and the Patient’s Personal and Sensitive Personal Information in accordance with this Policy and that any Personal Information provided by You through the Website is provided under a lawful contract.
Consent- By accessing and using the Website, or by providing Geri Care with Your Personal Information or by making use of the Services provided by the Company through the Website, you hereby consent to the collection, storage, usage, processing, disclosure and transfer of your Personal Information in accordance with the provisions of this Privacy Policy. You acknowledge that such consent is by your free will. You consent to forwarding Your information to a third party as needed to receive Services and You understand that existing confidentiality protections under this Policy apply.
- You consent to your Personal Information being used for clinical research by Geri Care after due de-identification and anonymization within the bounds of the prevailing laws applicable in India and this Privacy Policy.
- You acknowledge that while Services can be used to provide improved access to care, as with any medium of providing medical care, there are potential risks and no results can be guaranteed or assured. These risks include but are not limited to: technical problems with the information transmission or equipment failures that could result in lost information or delays in treatment. You understand that You have a right to withdraw Your consent to the use of Your Personal Information and we reserve the right to withdraw the provision of Services accordingly.
- You understand that We may store the Patient’s medical records after you cease to use our Services in an anonymized and de-identified manner such that your medical records will be redacted/ removed or obscured. We will use this information only for educational or for Our internal research and statistical purposes (including but not limited to market segmentation and customer value analysis).
The Company hereby states that they shall not distribute, disclose or share Your Personal Information like inquiries, chats, contact details to any third parties, without Your explicit consent except in exceptional circumstances, such as if disclosure is required by law or is necessary to protect the rights or property of the Company or any of its affiliates employees, agents etc., or to lessen a serious threat to a person’s health or safety. This may include if we have concerns that there is a serious threat to Your health or safety. - Patient’s Telephone Number or Registered User’s telephone number;
- Patient’s gender;
- Patient’s Date of Birth;
- Contact information of next of Kin;
- Transactional Information;
- Your preferences and opinions relating to the Website or any of our products and services;
- Pictures and other information from your device's camera and photo library;
- information that Your browser sends whenever You visit our Service or when You access the Service by or through the Website;
- To upload medical records, reports and files in various file formats such as ZIP, JPEG, PNG, Adobe PDF, Microsoft Word, DICOM (DCM) etc.;
- Your login and password as a Registered User;
- Records of your interactions with our Medical Practitioners;

Information we collect through Your use of the Website
- Location Information: We derive Your approximate location from Your IP address to fine-tune Your search for relevant Medical Practitioners, Geri Care Clinics, Geri Care Assisted Living Centres, Geri Care Hospitals, Geri Care offices, Pharmacies, Diagnostic centers and any other contact points through the Website, in order to optimize the use of our Services.
- Transaction Information: We may access transaction details related to Your use of the Website including the type of Services requested, date and time of the request, Geri Care Establishments from which Services have been availed, number of transactions conducted, fulfillment of the transactions, complaints against the Services availed and other related transaction details to (i) monitor the Website usage; and (ii) improve the Service offerings on the Website.
- Usage and Preference Information: We may access information about Your interactions with the Website such as the date and time You use the Website, the areas or pages of the Website or internet resources that You visit, the amount of time You spend viewing pages, the searches You make, the Medical Practitioner's profiles and the Service offerings You view; and the settings You have chosen. The Company uses cookies, to collect the information under this category. The Company may use the above-mentioned technologies to: (i) provide You with relevant content on Website as per your requirements; (ii) monitor the Website usage; and (iii) improve Services provided on and through the Website.
- Device Information: We may access information about Your device used to access the Website, including, for example, the hardware model, operating system and version, preferred language, unique device identifier, advertising identifiers, and browser information to facilitate the provision and resumption of Services and functions related to the Website, as well as software updates, and product support. - Log Information: When You interact with the Website, we collect server logs, which may include information like device IP address, access dates and times, and other system activity, type of browser, and the third-party site or service You were using before interacting with the Website to understand the usage of the Website.
- Interactive Features/Forums Information: The Company will store any information You share using the interactive features (such as audio calling facilities between You/ the Patient and the Medical Practitioner) listed on the Website. You understand that You are sharing information (including Personal Information and sensitive personal information) with clinics, physicians, medical specialists whilst using the Website and the Company guarantees security of such information as well as limit the nature of intended use of such information, to the best of its ability. You accept that any information (including personal information and sensitive personal information) which You may share with any third party application provider whilst using the Website, will be solely at Your risk as to the security of such information and that the Company only stores and does not specifically seek such information and cannot guarantee the security of such information or the nature of its use by the third parties to whom You disclose such information.

You understand and acknowledge that Your participation in such interactive features is completely voluntary and that You will share information (including Your/the Patient’s Personal and sensitive personal Information) with relevant Geri Care establishments, staff and personnel at its Call centers, Hospitals, Clinics, Assisted Living Centers, Home Care or other service providers of the Company, whilst using the Website. The Company both guarantees security of such information as well as shall limit the nature of intended use of such information, to the best of its ability. You accept that any information (including personal information) which You may share with any third party application provider whilst using the Website, will be solely at Your risk as to the security of such information and that the Company only stores and does not specifically seek such information and cannot guarantee the security of such information or the nature of its use by the third parties to whom You disclose such information.
Disclosures of Personal & Other InformationThe Company may share or transfer information You disclose on or is collected through the Website only for the following purposes:
- Medical Practitioners that are empanelled by Geri Care.
- Our affiliates, including but not limited to other organisations and businesses within our group of organizations and as permitted under the applicable law
- to identified partners for their business use (including the supply of Services requested by You), when You subscribe to Services of the Website, Service request orders which are made available through the Website or seek information specific to them;
- as required by law or in the interest of protecting or exercising the Company’s or others' legal rights, e.g., without limitation, in connection with requests from law enforcement officials and in connection with court proceedings; and
- to partners of the Company who specialize in Big Data analytics, marketing and advertising, developers of Software required and relevant for providing services and for ongoing research and development by Geri Care or its Partners, Vendors, associates and affiliates, who shall use the data in their businesses, on an anonymized and de-identified basis such that it cannot reasonably be used to identify You.
The Website are intended only for adults and NOT for minors (individuals under the age of 18). The Company does not intentionally or knowingly collect personally-identifiable information from minors and minors must not submit any personal information on the Website. The parents or the legal guardians of such minors must explicitly make clear to the Company that the Personal Information being shared with the Company related to that of a minor and provide express consent for the processing of the same.
